Who can qualify for a Medical Cannabis Card?

Individuals with the following conditions are authorized under the Utah Medical Cannabis Act to receive a medical cannabis card:

Qualifying Conditions

  • Alzheimer’s disease

  • Amyotrophic lateral sclerosis

  • Autism

  • Cancer

  • Cachexia

  • Crohn’s disease or ulcerative colitis

  • Epilepsy or debilitating seizures

  • HIV or acquired immune deficiency syndrome (AIDS)

  • Multiple sclerosis or debilitating muscle spasms

  • Persistent nausea (Cannot be related to pregnancy, cannabis induced cyclical vomiting syndrome, or CBD hyperemesis syndrome)

  • Persistent Pain

  • Post-traumatic stress disorder

  • Terminal illness when the patient’s remaining life expectancy is less than 6 months

  • A condition resulting in the individual receiving hospice care

  • Rare condition or disease that affects less than 200,000 individuals in the U.S., as defined in federal law and this is not adequately managed despite treatment attempts using conventional medications (other than opioids or opiates) or physical interventions

    What do we do?

    We are not a medical cannabis dispensary and we do not sell medical cannabis in any form. We provide appointments with our QMP (qualified medical provider) to help Utah residents qualify for a medical cannabis card.

    How long does it take?

    The process starts with scheduling an appointment with our QMP. You will then receive an email with pre-appointment instructions.

    The appointment itself takes between 30 to 60 minutes. If our QMP determines that you qualify for a medical cannabis card you will leave the appointment with a recommendation already submitted to the state, the state will issue you a card. It typically takes up to two weeks to receive your card.

    What is the cost?

    Intake: $150

    • 30-60-minute in-person new-patient appointment as required by the State of Utah

    Renewal: $75

    • 20-minute Telehealth or in-person appointment

    Intake & Compassionate Use Board (CUB) application: $200

    • 30-60-minute in-person new-patient appointment as required by the State of Utah

    and an application to CUB for any person less than age 21 as required by the State

    of Utah to request permission for use from the State of Utah.
